If you are unable to get to see us at the Clinic it is still possible to benefit from our services. All you need to do is to spend some time completing our Questionnaire and to post it to us. This can be downloaded and printed by pressing on the appropriate menu button in the left-hand column of this page. Please don't be put off by the length of it - it is much easier to complete than you might first imagine and you don't need to worry if you are unable to answer all of the questions - just fill in what you can. The more information we have the more accurately we can interpret your answers.

Upon receipt of this we will study your replies and put together diet sheets and supplements tailored to you personally. But we won't just leave you to put them into effect. The price of this service will also include a starter Menu plan to help you implement it plus a follow up email correspondence if required to answer any queries you may have. If you need further help, this can be arranged by contacting us.

The cost of this service is just £35.00, payable by cheque (made payable to Christine Anderson) which should be sent with your Questionnaire when you return it to the address below:
